First-Generation Versus Second-Generation Antipsychotics in Adults: Comparative Effectiveness Executive Summary
ثبت نشده
چکیده
Antipsychotic medications are used to treat and manage symptoms for several psychiatric disorders and are commonly categorized into two classes. First-generation antipsychotics (FGAs), also known as “typical antipsychotics,” were developed in the 1950s. Secondgeneration antipsychotics (SGAs), also known as “atypical antipsychotics,” emerged in the 1980s. To date, FGAs have been classified according to their chemical structure, which includes serotonin-dopamine antagonists and multiacting receptor-targeted antipsychotics, whereas SGAs have been categorized according to their pharmacological properties as dopamine partial agonists. There is ongoing research testing the proposed mechanisms of action within each class with respect to the neurobiology of different psychiatric disorders.1,2

Second-Generation Antipsychotics and Extrapyramidal Adverse Effects
Antipsychotic-induced extrapyramidal adverse effects are well recognized in the context of first-generation antipsychotic drugs. However, the introduction of second-generation antipsychotics, with atypical mechanism of action, especially lower dopamine receptors affinity, was met with great expectations among clinicians regarding their potentially lower propensity to cause extrapyramidal syndro...
متن کاملAntipsychotics for children and young adults: a comparative effectiveness review.
BACKGROUND AND OBJECTIVE Despite increasing on-label and off-label use of antipsychotics, prescribing antipsychotics to children remains controversial due to uncertainty of their relative benefits and safety. We systematically reviewed the effectiveness and safety of first- (FGA) and second-generation antipsychotics (SGA) for patients aged ≤24 years with psychiatric and behavioral conditions. ...
متن کاملPropensity Score Estimation to Address Calendar Time-Specific Channeling in Comparative Effectiveness Research of Second Generation Antipsychotics
BACKGROUND Channeling occurs when a medication and its potential comparators are selectively prescribed based on differences in underlying patient characteristics. Drug safety advisories can provide new information regarding the relative safety or effectiveness of a drug product which might increase selective prescribing. In particular, when reported adverse effects vary among drugs within a th...
متن کاملCost-Effectiveness of Paliperidone Palmitate Versus other Antipsychotics for the Treatment of Schizophrenia in France.
METHODS • Schizophrenia is a severe mental disorder, characterized by profound disruptions in thinking and affecting language, perception, and the sense of self.[1] This disabling and emotionally devastating illness is manifested by ‘positive’ (e.g. delusion, hallucinations) and ‘negative’ (e.g. blunted affect, alogia) symptoms. • In 2005, the prevalence of schizophrenia patients in France was ...
متن کاملClinical trial-based cost-effectiveness analyses of antipsychotic use.
OBJECTIVE Second-generation antipsychotics make up one of the fastest growing segments of the rapidly growing pharmaceutical sector. Given limited health care resources, assessment of the value for the cost of second-generation antipsychotics relative to first-generation antipsychotics is critical for resource-allocation decisions. METHOD With a MEDLINE search, the authors identified eight st...
